Practice information: CNS, established in 1991, is an outpatient clinic providing evaluation and treatment for patients of all ages with acquired brain injuries, primarily mild traumatic brain injuries. With a team of nursing, occupational and speech language therapists, comprehensive assessments are conducted which guide treatment planning/goal setting. Detailed neuromedical evaluations utilizing 3.0 Tesla MRI and volumetric analysis, neuroendocrine studies, nutritional studies, sleep evaluations, audiologic evaluations, neuro-optometric evaluation and physical therapy/vestibular evaluations define the baseline deficits requiring stabilization to optimize readiness for neurorehabilitation. CNS has employed PAs and NPs for over a decade as critical team members in this process

Position information: This is a full time permanent position in an outpatient setting, Monday through Friday, with no weekend clinic operations/duties. Once the PA has been adequately trained, on-call (phone) duties will be assigned for weeknights and weekends, but always with an attending MD as backup. PA, by deliberate design, will not be assigned on call duties for major holidays. No inpatient or admitting privileges required. Maximum patients seen during day 10-15.

Brief job description: PA will be expected to examine, evaluate and treat with the assistance of the Medical Director and treatment team both new patients and current patients of CNS. A strong structured onboarding process involving close personal supervision is provided in the initial stages of employment to assure adequate mastery and knowledge base of foundations of brain injury medicine (neurology, neuropsychiatry, neurorehabilitation) and readings/articles will be provided for education. As mastery is demonstrated, increasing levels of independence will occur with continual Medical Director availability to assure PA comfort and patient's quality of care demands. Our Senior PA will also provide guidance and facilitate professional development (herself an EVMS graduate).

Experience Required: Will train a new grad, or will accept a PA with prior experience in a relevant field.

In This Role:

• We will be looking for a minimum of 2 years' experience in Cardiothoracic Surgery as a board-certified licensed Physician Assistant. You will work under the direction of a Cardiothoracic Surgeon and within established departmental practice guidelines, this Advanced Practitioner is responsible for performing a variety of duties.
• Responsibilities will include but are not limited to pre-op evaluation of patients and preparation for surgery, first and second assisting in cardiac and thoracic surgery, endoscopic vein and radial artery harvesting for bypass, as well as bedside assisting for robotically assisted cases. Additionally, the candidate must be able to make rounds in the SICU and telemetry floor, efficiently progressing the patient toward discharge. The candidate must be proficient at removing chest tubes, pacing wires, providing wound care, general procedures utilizing local anesthesia, new consults and discharging patients as well as discharge summaries. Experience with managing mechanical assist devices including durable LVAD, Impella and ECMO is desirable but not required. Willingness to learn central line and arterial line placement, chest tube placement and point of care ultrasound is also required.

The APP will practice in an outpatient neurology setting with a focus on memory disorders, general neurology, and longitudinal neurological care. A major portion of this role is within Riverside’s Comprehensive Memory Care Center, embedded in Neurology. The Memory Care Center is an APP-led model supported by social workers, pharmacy medication reviewers, RNs, and ancillary staff, providing clinical and caregiver support across medical, behavioral, and psychosocial domains. Work collaboratively with neurocognitive specialists and geriatricians at the Martha Goodson Center to deliver coordinated dementia care. 

CORE SCOPE OF PRACTICE

• Diagnose and manage patients with general neurological conditions, including dementia, epilepsy, headache, and sleep disorders
• Perform neurological examinations and assessments
• Order, and collaboratively interpret, and act on diagnostic testing (laboratory, neuroimaging, EEG, EMG)
• Develop and implement individualized treatment plans
• Prescribe medications and review therapeutic responses
• Provide patient and caregiver education and counseling
• Document care using Epic EMR with integrated AI documentation and messaging tools
• Collaborate closely with neurologists, nursing staff, pharmacists, social workers, and care coordinators

SCOPE OF GENERAL NEUROLOGY AND SUB-SPECIALTY PRACTICE: While the primary focus remains general neurology and dementia care, the APP may gain additional experience in the following areas based on interest: 

• Movement Disorders: exposure to advanced therapies including MR-guided focused ultrasound (MRgFUS), deep brain stimulation (DBS), and foslevodopa pump systems (Duopa, Vyalev). The program also participates in clinical trials.
• Memory Care: Alzheimer's serum biomarkers, MRI, and brain PET imaging, and coordinating access to on-site anti-amyloid antibody therapies for Alzheimer’s disease. Neurocognitive (psychology) testing is available onsite.
• Infusion Therapies: Multiple sclerosis and myasthenia disease-modifying therapies, Alzheimer’s monoclonal antibodies, IVIG, and migraine biologics (administered in an infusion center staffed by trained neurology RNs)
• Comprehensive Stroke Program: collaboration with neurovascular neurosurgeons, neurocritical care specialists, and hospitalist teams, Neurophysiology & Sleep Medicine: access to in-house EEG, EMG, and accredited sleep laboratory services
