OES Vehicle Policy Participant Form

To ensure liability coverage during field activities with student classes and volunteers, we now have a single form to provide risk assumption for the department.

This form will need to be signed by:

-Each participant of a class field trip or activity that is conducted off campus. This includes participants who are providing their own transportation.

-Volunteers that participate in any field or sea activity through the department either as a classroom activity or research assistant of faculty.

*NOTE* 108 must be submitted to the department office. DO NOT BRING THEM TO THE RESEARCH FOUNDATION. Submit your forms 4 days prior to the 108 Processing Due Date. If you do not bring it to the office, your tuition will NOT be paid on time.
